A Privacy Breach occurs when someone accesses information without permission.  This information may include personally identifiable information such as a first/last name or birthdate. 


When confidential client information is recorded in the HMIS ServicePoint, it is the responsibility of us all, as HMIS users, to ensure the highest level of diligence to protect this information from being viewed by unauthorized individuals.      


As clients move between programs within Calgary's Homeless Serving System of Care and client information in HMIS is shared between programs, we should always be looking out for instances where Client information could be visible when it shouldn't be.  


If you think you are viewing a client profile in ServicePoint that you shouldn't be able to see (is not in your program) OR you suspect for any reason that your Client's information has been breached in any way, immediately report this to your supervisor and to the HMIS Support Team at hmis@calgaryhomeless.com.
