Program Transfer-CAA is an extension of CAA (Coordinated Access & Assessment), and acts as the virtual holding program for all Housing First (HF) clients who have been flagged for a transfer from one HG program to another. These clients are populated on the Program Transfer tab of the CAA Triage lists and are discussed by the CAA table for placement in the new program as per each table's Terms of Reference.
Who will the client's information be shared with upon completing a referral to Program Transfer - CAA?
- Your client's first and last name, and demographics as well as information entered into the "Housing First Program Transfer Application" assessment, will be provided in a triage dashboard for all of the Placement Committee members to see. If your client is triaged to a program which utilizes Calgary's HMIS, this information will then be shared with them.
Who has access to Program Transfer - CAA in HMIS?
- Calgary Homeless Foundation's System Planners and the HMIS Support Team.
Why do we need to have a valid CAA ROI for Program Transfer - CAA?
- As referred clients are discussed and triaged through the CAA (Coordinated Access and Assessment) process, and therefore CAA participating partners may be privy to the information entered about a client in need of a transfer, the most current CAA ROI is the best document to explain and request consent for this collaborative process.
