Most likely, your case notes are no longer visible to you because the “Provider” was changed on the Goal (case note folder) where you recorded your case notes. Changing this “Provider” from the original program which created the goal causes an issue with visibility.
Contact the HMIS team at hmis@calgaryhomeless.com to correct this issue by returning the Provider back to the original creating program.
To avoid the issue of disappearing Case Notes, going forward please only enter your notes in a Goal folder that was created by your program/provider. If you do not see your program listed as the Provider, please create a new Goal (case note folder) for the correct Provider/Program.
