The Housing First Exit Assessment is client centric, which means it should be filled out from the client’s point of view. At the beginning of the assessment, you can indicate whether the client was available to complete the assessment with you or not:

If you select Yes to this question, you must have the client answer the Satisfaction Survey at the end.
