On the Housing First Move in and Housing First Quarterly Assessment, there is a health section and a legal section near the end of the assessment. There are three different ways to answer these questions:

1. If the participant states they have not had any health or legal system involvement, you should answer No, followed by 0’s in the following boxes.
2. If the participant states they have had health or legal system involvement you should answer Yes, followed by at least 1 number in one of the following boxes. You cannot answer Yes followed by 0’s. Yes indicates that the participant will have a number greater than zero in the following boxes.
3. If the client states they do not know the answer to the question, please leave the following four boxes blank.
