Addictions Treatment | A substance use disorder treatment program, detox program, or other substance abuse residential facility. |

Age-Related Supports | A supportive living environment that combines accommodation services with other supports and care for seniors and people with disabilities. |
|
Children's Services Placement | Children or youth who are brought into temporary or permanent care and are placed in an alternate placement until they are able to return to a permanent home. |
|
Couch Surfing / Provisionally Accommodated | Couch surfing or sofa surfing is the practice of moving from one house to another, sleeping in whatever spare space is available, floor or couch, and generally staying a few days before moving on to the next house. |

Death | Participant has passed away. |

Declined to Answer | Participant not willing to provide information. |
|
Emergency Shelter | Staying in overnight shelters for people who are experiencing homelessness. |
|
Health: Hospital or Medical Facility | The participant has exited to a hospital for any reason, including for psychiatric support. Includes any residential care involving a medical need. |

Health: Long-term Care | Long term care facilities are designed specifically for individuals with complex medical needs who require 24-hour onsite Registered Nurse assessment and/or treatment. |

Hotel/Motel | A temporary arrangement where occupants pay money, but the accommodation does not offer the possibility of permanency and instead is based on a nightly/weekly basis. |
|
Justice System | The participant has been arrested and is incarcerated in a remand facility, prison, or juvenile detention facility. |
|
Natural Supports | Reconnection with individuals who the participant defines as a positive natural support in stable housing. |
|
No Contact | Program has not been able to reach participant for services or contact/communication for a period of time sufficiently extended to warrant program exit |
|
Non-Market Affordable Housing | Independent housing where there is an investment by a level of government, private business, or non-profit organization that allows the cost of that housing to be offered to renters or owners at a price that is less than the current market value. Case management is either not provided or external to the housing provider. |

PDD Placement | The Persons with Developmental Disabilities (PDD) program helps eligible individuals plan, coordinate, and access services to live as independently as they can in their community. |
|
Long Term Place-Based Supportive Housing (PBSH) | Case management and housing supports for individuals with no time limit. Placements are designated to specific buildings and/or locations, often with agency supports on site |

Renting - Unsubsidized | Properties within the unsubsidized market are regular private rentals at rental rates comparable to the surrounding market. Unsubsidized rentals do not offer direct or indirect subsidies. |
|
Rough Sleeping / Dwelling unfit for human habitation | Rough sleeping can refer to anyone who is unsheltered, sleeping in parks, vehicles, or squatting in derelict buildings for temporary shelter. |
|
Long-Term Scattered Site Supportive Housing (SSSH) | Case management, rent subsidy and housing supports for individuals and families with no time limit. Placements are not designated to one specific building, rather individuals and families are housed in the community. |

Short Term Housing | Housing placement that is an intermediate step between emergency shelter and permanent housing. |
|
Second Stage Shelter | Second stage housing programs provide housing and support for women who have left abusive relationships as they make plans for independent living. |
