List of Participating Programs
As a Participant of the Outreach Collaborative, your personal information is stored in Calgary’s Homeless Management Information System (HMIS). This allows that information to be shared among outreach programs in Calgary to coordinate services to best fit your needs. Information collected through the HMIS will be electronically visible to any outreach program that is a part of the Coordinated Outreach. The programs include

None of the information collected will be disclosed to any parties outside of the Coordinated Outreach, except if required by law or specifically consented to by you, the Participant. Consent will be valid for a period of three (3) years from the date of signing, or until you have withdrawn my consent, whichever comes first.
