When completing the Housing information sub-assessment:

Please ensure you are back-dating to the date the original CHF Housing Assessment was completed.
For all subsequent CHF Housing Assessments (in situations where the participant was re-housed or transferred within your program), please also back-date to the date the CHF Housing Assessment was completed.

When completing the eviction information sub-assessment:

For SSSH: Please ensure you are back-dating to the date the participant was evicted each time from their housing.
For PBSH: Please ensure you are back-dating to the date the participant was evicted from your program, which will equal the participant's date of exit from your program.
