Assessments that only exist in HMIS do not have hyperlinks. You must access by logging into HMIS. 


Definitions:

  • Intake Date: date the participant/client agrees to work with your program and/or accept services (also known as Program Entry Date)
  • Move In Date: date the participant/client physically moves into their unit
  • Program Entry Date: date the participant/client agrees to work with your program and/or accept services (also known as Intake Date)
